Description:The Supportive Case Management Intern will engage with Delores� Safe Haven participants to provide/cultivate case management and therapeutic services. Participants are women ages 18 and up, without children in their care, have an Axis I mental health diagnosis and a history of chronic homelessness.
Duties:Primary Tasks and Responsibilities:

� Frequent engagement with clients, spanning all housing programs in the building. The nature of these interactions will differ based on client history, need, and current situation, as well as her respective program. The intern will work to expand and elaborate on services provided to participants and tenants.

� Create/develop and facilitate at least one weekly group for participants, residents and tenants in any or all of the 3 housing programs.

� Assist Case Managers with relevant tasks, including research of resources and relevant clinical information, meetings with participants/tenants, paperwork completion incl. case notes and goal plans, filing, etc.

� Participate in bi-weekly seminar-style discussions with the Case Management team about a student- or collectively selected article or chapter that relates to the intern�s work at Deborah�s Place.

� Participate in weekly supervision.

� Participate in on-site training and staff development opportunities on a consistent basis.

� Participate in weekly or twice-monthly �Clinical Rounds� meetings
Qualifications:Applicant Strengths and Qualities:

� Interest in frequent, consistent engagement with clients

� Strong interest in women�s issues, and in working with women who have experienced homelessness and may have dealt or are dealing with mental illness, substance abuse, trauma, etc.

� Self-directing and comfortable working independently, without constant direct supervision

� Interest in Harm Reduction and the Recovery Model philosophies


The Safe Haven Case Manager engages with the 15 Safe Haven participants and PCA tenants on a daily basis, specifically seeking out participants that are difficult to engage. Interactions should be therapeutic and reflective of the participant�s goal plan. The CM follows up with participant immediately after a violation of building rules and guidelines. CM maintains a general presence in the building, spending time in the Safe haven community space, CM office and PCA tenant lounge. CM completes staffing on participant/tenants, maintains charts and documentation, making sure that chart notes reference goal plans; enters data into HMIS by the end of the week; files paperwork/notes in charts; completes Goal Plans with the participant every six months; plans and facilitates groups, activities and field trips, etc.
