| Duties: | The primary work of a case manager at Deborahs Place is to work individually with participants to overcome the obstacles that they face while trying to achieve their goals. It involves being an advocate, a trusted and reliable confidante, a resource advisor, and always being the participants� �go to� person during a crisis. |
| Qualifications: | Maintain a caseload of Alumnae; women who are housed but not living in DP housing, and who attend the Learning Center for day time services.
Conduct intakes and assessments with alumnae.
Conduct service planning with participants (helping them to identify their goals and the steps they will take to reach them.
Provide necessary resources and referrals to participants in order to help them achieve their goals.
Maintain participant files, record progress notes, and enter service-related information into agency database.
Act as an advocate for alumnae on case load.
Provide support, encouragement, and objective feedback to participants. Observe progress, make adjustments to service plan as goals are achieved and/; or changed.
Facilitate, or co-facilitate, an ongoing group or activity (topic of your choice).
Participate in weekly supervision with Director of Programs. Participate in staffing with other interns and staff and attend on-site agency trainings or workshops that are offered during internship.
Work as a team member with Learning Center staff, other building staff, supervisor and the agency as a whole toward the agency-wide goal of helping alumnae maintain housing and achieve personal goals.
